Navel and midline

Weak points in the centre of the abdomen.

At the navel, the abdominal wall has been thinner since birth: this is where the umbilical cord ran, and the umbilical ring remains a weak point for life. If tissue pushes outwards through this gap, the hernial orifice, an umbilical hernia develops – no hernia of the front abdominal wall is more common. The epigastric hernia lies higher up, in the midline between the navel and the breastbone, where the tendon sheets of the abdominal muscles meet.

Both show up as a bulge that emerges when coughing, straining or sitting up from lying down, often accompanied by a pulling sensation on exertion. The epigastric hernia is usually small and can still be quite painful, because fatty tissue becomes trapped in the narrow gap.

Such hernias are favoured by excess weight, pregnancy and anything that raises the pressure in the abdomen over the long term, such as a chronic cough or frequent straining due to constipation. Sometimes the straight abdominal muscles also separate in the middle at the same time, a condition known as rectus diastasis.

In babies, an umbilical hernia often closes by itself in the first years of life, but in adults it no longer does. Tissue can become trapped even in a small gap – this is easily underestimated. Whether and when to operate depends on symptoms, size and risk.

Mesh from the inside

As a rule, a special mesh is placed against the abdominal wall from the inside through an open incision at the hernia – the IPOM technique. It bridges the gap over a wide area.

Direct suture

The gap is sewn closed without a mesh. Because the hernia recurs more often afterwards, this option is reserved for selected cases.

Outpatient possible

Depending on the size of the hernia and your health, without an overnight stay or with an inpatient stay. This is decided in the preliminary consultation.

The path

From diagnosis to closure.

How the operation is performed depends on the size of the gap, your symptoms, the tissue and your health.

  1. Examination

    Palpation lying down and while tensing the abdominal muscles; an ultrasound measures the hernial opening and shows any rectus diastasis.

  2. Planning

    The procedure, the anaesthesia and whether the operation is done as an outpatient or inpatient are decided together.

  3. Surgery

    Open with a mesh using the IPOM technique or, in selected cases, with sutures.

  4. Going home

    After outpatient anaesthesia, an accompanying person collects you; you must not drive yourself for 24 hours.

  5. Aftercare

    Wound checks and movement in everyday life. Abdominal training and heavy lifting only follow after medical clearance.

If the hernia needs an operation, German statutory health insurance covers the costs. No prior approval is required; a referral from your GP is usually enough for the first consultation.

Why a mesh if the gap is small?

Registry studies and randomised trials show that an umbilical hernia recurs much less often after mesh reinforcement than after a simple suture. This also applies to hernial openings of one to two centimetres and when a rectus diastasis is present at the same time.

What is a rectus diastasis?

A separation of the two straight abdominal muscles in the middle, often after pregnancy. It is not a hernia in itself, but it can occur together with an umbilical hernia and then speaks in favour of a mesh.

Can I go home the same day?

That is possible if the findings allow it, there are no serious pre-existing conditions and someone is at home with you. Larger hernias or other conditions speak in favour of an inpatient admission.

I would still like to have children – what does that mean?

Pregnancies stretch the abdominal wall and favour umbilical hernias. If you are planning a pregnancy, you should mention it in the preliminary consultation; it can influence the timing and the procedure.

What are the risks?

As with any operation, bleeding, wound infections and healing problems; wound fluid can collect under the skin. If the mesh lies inside the abdominal cavity, adhesions are possible. A recurrent hernia is less common with a mesh, but not ruled out.

When do I need to go to hospital immediately?

If the bulge suddenly stays firm, is very painful and nausea or vomiting set in. Tissue can become trapped, especially in narrow hernial openings – then go to an emergency department without delay or call 112.

Do I need to apply to my health insurer?

No. Surgery for an umbilical hernia is a standard benefit if it is medically indicated; the hospital bills the insurer directly.
