Arthroscopy

Looking inside the joint without opening it.

In an arthroscopy, the surgeon inserts a rod-shaped camera, the arthroscope, into the joint through a small incision. Fine instruments are introduced through one or two further portals. A clear rinsing fluid expands the joint, and a magnified image appears on a monitor.

Today, most questions are answered beforehand by ultrasound, X-ray and magnetic resonance imaging (MRI). Arthroscopy is therefore used mainly for treatment: in the knee, for example, for a trapped piece of meniscus, loose bodies or limited cartilage damage; in the shoulder, for calcific tendinitis or narrowing beneath the shoulder roof (impingement); in the ankle, for impingement caused by bone or scar tissue.

It is most useful when a mechanical problem explains the symptoms – a joint that locks, catches or grinds. In general wear and tear, meaning osteoarthritis, and in degenerative meniscus tears, targeted exercise therapy usually helps just as well as surgery.

An arthroscopy is smaller than an open procedure, but it is still an operation – with anaesthesia, a consent discussion and aftercare. How long recovery takes depends less on the size of the incisions than on what was done inside the joint.

The path

From the question to the arthroscopy.

The first question is whether an arthroscopy will really help. That is why the path begins with an examination and imaging, not with a date for surgery.

  1. Preliminary consultation

    Please bring your health insurance card, a referral if you have one, a list of your medications and previous results such as X-ray or MRI images. The orthopaedic examination checks mobility, stability and tenderness.

  2. Weighing up

    Together you discuss whether physiotherapy or an injection into the joint will be enough, or whether arthroscopy is the better option.

  3. Anaesthesia

    The procedure is carried out under general anaesthetic or with a regional anaesthetic that numbs only the leg or arm. Whether you go home the same day or stay overnight is decided by the doctor, based on the procedure and your state of health.

  4. Procedure

    Through two or three small portals, the joint is rinsed, examined and treated. At the end, a stitch or an adhesive strip closes each incision.

  5. The days after

    Cooling, elevation and early exercises with the physiotherapist reduce the swelling. After knee procedures, crutches take the weight off the leg; after shoulder procedures, a sling supports the arm.

  6. Follow-up

    At the follow-up appointment, the wounds and mobility are checked and the findings from the joint are discussed. Physiotherapy then builds up strength again.

Statutory health insurance

At the Bürgerspital, with one exception.

If an arthroscopy is medically indicated, German statutory health insurance covers the costs, both as an outpatient and as an inpatient; the procedure is then carried out at the Bürgerspital Wertheim. The exception is rinsing and smoothing the joint for osteoarthritis of the knee: the Federal Joint Committee (G-BA) removed these from the benefits catalogue in 2015 because studies showed no benefit.

Even with osteoarthritis, statutory insurance covers the arthroscopy if an injury, an acute locking of the joint or meniscus damage clearly explains the symptoms. Before arthroscopic procedures on the shoulder, patients with statutory health insurance are also entitled to a second medical opinion.

Private insurance or self-pay

Under your policy or at your own expense.

For patients with private health insurance and for those covered by Beihilfe (the German civil service medical allowance), reimbursement depends on the individual policy.

Patients with private insurance and self-pay patients can choose between the Bürgerspital and the Main-Tauber-Klinik, a private clinic under the same roof.

Is arthroscopy done as an outpatient procedure?

Often, yes, especially smaller procedures on the knee. Whether an overnight stay makes sense depends on the type of procedure, any existing conditions and the care available at home. After outpatient surgery under anaesthesia, please arrange for someone to collect you and do not drive yourself.

What kind of anaesthesia will I have?

Usually a general anaesthetic or a regional anaesthetic. For the knee, a spinal anaesthetic is often enough; for the shoulder, a nerve block of the arm may be added, which continues to relieve pain after the operation.

How long will I be restricted afterwards?

That depends on what was done in the joint. After smaller procedures such as smoothing the cartilage, many people are back at work after two to four weeks. If something was stitched – a meniscus, cartilage or a tendon – the tissue needs weeks to months of rest.

What can go wrong during an arthroscopy?

Complications are rare but possible: joint infections, in around one in a thousand procedures, bruising, persistent fluid in the joint, thrombosis and, very rarely, injuries to nerves or blood vessels. After procedures on the leg, anticoagulant injections are often given to prevent thrombosis.

What is calcific tendinitis of the shoulder?

Calcium deposits form in one of the shoulder tendons and can cause severe pain. They often recede on their own or with non-surgical treatment. If the symptoms persist, the deposit can be removed arthroscopically.

What can be treated by ankle arthroscopy?

Common reasons are impingement caused by bony spurs or scar tissue at the front of the joint, loose bodies and limited cartilage damage on the talus. A permanently inflamed joint lining can also be removed this way.

Is treatment carried out during the arthroscopy?

Usually, yes. What is planned is discussed beforehand, and the consent discussion determines what may be done if an unexpected finding arises. Some things are then treated later in a separate procedure.
