The nose
The nose sits in the middle of the face and helps to determine its proportions. At the same time, it is the natural path for the air we breathe: it warms, moistens and cleans the air before it reaches the lungs. People who breathe poorly through the nose often sleep more restlessly, snore or switch to breathing through the mouth.
Common causes are a deviated septum, a crooked nose after an injury or weak nostril walls that collapse when breathing in. Other people are mainly bothered by the shape: a hump, a wide or long nose, a bulbous or drooping tip.
Often the two are linked. Changing the external shape also changes the inside of the nose – which is why breathing is part of every plan. A correction that would impair function does not make sense.
The treatment
A straightened nasal septum and a stable nasal framework can improve nasal breathing.
The hump, tip, width and length can be tailored to the face – with the aim of a natural-looking result.
Noses that have been injured or operated on before, or that have congenital differences, can still be corrected in many cases.

Every rhinoplasty starts with two questions: what should the nose be able to do – and which shape suits this face?
A specialist in plastic and aesthetic surgery examines the shape and breathing, listens to your wishes and explains what is possible and what is not.
The technique depends on the findings: closed, with incisions inside the nose, or open, with a small incision on the columella, the strip of skin between the nostrils. If breathing is obstructed, the septum is straightened in the same operation.
Cartilage and bone are carefully reshaped. The procedure usually takes one to three hours and is generally performed under general anaesthesia; smaller corrections are also possible under local anaesthetic.
A splint supports the nose, and cooling eases the swelling. Most patients go home after one to two nights.
The splint is removed after one to two weeks, the stitches after about 14 days. Sport, sun and glasses are put on hold for a while; the final result becomes apparent after a few months.
Follow-up appointments accompany the healing process and check shape and breathing.
Statutory insurance or self-pay
Statutory health insurance
If nasal breathing is obstructed, for example by a deviated septum, the correction may be medically necessary. Whether German statutory health insurance covers this part is assessed case by case. It does not cover the aesthetic reshaping.
Operations covered by statutory insurance take place at the Bürgerspital Wertheim; pre- and post-operative care is also possible close to home at the other locations in Germany.
Private insurance or self-pay
Private health insurance and Beihilfe (the German state healthcare allowance for civil servants) may partly cover the functional part; patients pay for the aesthetic part themselves.
The costs depend on the extent and technique: correcting the tip is a smaller procedure than a full rhinoplasty, while a functional part or previous surgery makes the procedure more complex. You receive your personal quote after the preliminary consultation; several clinics offer financing in instalments.

German statutory health insurance may cover the functional part – such as straightening the septum when breathing is obstructed – if it is medically necessary; this is assessed case by case. The aesthetic correction is a self-pay treatment. Private health insurance and Beihilfe partly cover the functional part.
This is the name for septoplasty and rhinoplasty combined: the septum is straightened and the external shape corrected in the same operation. Breathing and shape are thus treated together.
The procedure itself is performed under anaesthesia. Afterwards, pressure, tightness and pain can be felt, but these can be treated well with medication. In the first few days, nasal breathing is restricted by the swelling.
With the closed technique, all incisions are inside the nose. With the open technique, a fine scar remains on the columella, which usually heals inconspicuously.
Usually after one to two weeks, once the splint has been removed. Most people take about two weeks off work. Slight swelling, especially at the tip of the nose, may remain for a few weeks.
As with any operation, bleeding, infection and wound-healing problems are possible. In addition, there may be asymmetry, altered skin sensation and, rarely, impaired nasal breathing; sometimes a follow-up correction is needed. The informed-consent discussion goes through each point individually.
An aesthetic correction only once the face has finished growing. Functional procedures may be medically necessary earlier – the medical examination decides.
In many cases, yes. After previous procedures the tissue is scarred, and sometimes cartilage is missing and has to be replaced. Such a correction is more complex and is planned with particular care.
