The starting point
Where the body stores fat is largely down to predisposition. In women, it is often the hips, thighs and buttocks; in men, the abdomen and flanks. Such deposits often barely respond to exercise and diet, even when weight is within the normal range.
Liposuction changes the shape of one area in a targeted way. It is not a method for losing weight: it is best suited to people of roughly normal, stable weight whose skin can settle back into place afterwards. For people who are severely overweight, it is not the right approach.
Not every accumulation of fat on the legs is a question of shape. Lipoedema, a chronic disease of the fatty tissue, has its own treatment pathway with its own requirements. It is described on a separate page.
The treatment
Abdomen, waist, hips, thighs, knees, upper arms, back or chin: the area that is out of proportion is treated – several in one session, where this is medically justifiable.
Fat cells that have been removed do not grow back. With a stable weight, the new contour is maintained; if you gain a significant amount of weight, the remaining cells can grow.
Tumescent technique with fine cannulas, water-jet-assisted or vibration-assisted depending on the clinic (WAL, PAL). The access points are only a few millimetres in size.

Liposuction is an operation. That is why the first question is whether it is the right approach – only then does the planning follow.
A specialist examines fat distribution and skin quality and discusses which areas make sense and what can realistically be achieved.
After consultation with your doctor, blood-thinning medication is paused about two weeks beforehand. Alcohol and nicotine should be avoided as far as possible in the days before.
On the day of the procedure, the areas are marked while you are standing – the way the contour looks in everyday life.
Under local anaesthesia, twilight sedation or general anaesthesia, a solution of saline and anaesthetic is introduced. Through small incisions, for example in the navel, fine cannulas loosen the fat and suction it out.
A compression garment is put on immediately afterwards. It is worn for four to six weeks, day and night at first, and counteracts swelling.
Swelling, bruising and a feeling like muscle soreness are normal in the first few days. Sitting for more than two hours at a stretch should be avoided because of the risk of thrombosis, as should saunas and intense sun.
The new contour emerges as the swelling subsides. It can be judged definitively after a few months, and on the thighs sometimes only after six to twelve months.

Liposuction for aesthetic reasons is not covered by German statutory health insurance. The cost depends mainly on the number of zones, as well as on the length of the procedure, method, anaesthesia and stay.

For lipoedema
In lipoedema, pathologically altered fatty tissue is removed to relieve symptoms – a treatment, not a correction of shape. Under certain conditions, it is covered by statutory health insurance.

No. Liposuction shapes individual areas; it hardly reduces body weight and is no substitute for diet and exercise. It is best suited to people of roughly normal, stable weight. The clinics advise against it for people who are severely overweight.
The basis is the tumescent technique: a solution of saline and local anaesthetic loosens the fatty tissue. Depending on the clinic, the fat is removed with water-jet assistance (WAL) or with vibrating cannulas (PAL). Which technique is right depends on the area and the findings.
This is limited for medical reasons and depends on the area and your state of health. As a guide for one procedure, the Alster-Klinik quotes around four litres. Several zones in one session are possible, where this is medically justifiable.
The access points are only two to five millimetres in size and are placed in inconspicuous locations, such as the navel. After the healing phase of two to three months, they are usually visible only as small dots.
Fat cells that have been removed do not grow back. With significant weight gain, however, the remaining fat cells can grow – in other areas too. A stable weight keeps the new contour in place.
The skin needs time to adapt to the new shape; on the thighs, six to twelve months is not unusual. If it has little elasticity, liposuction can be combined with Renuvion® or with a lift of the abdomen, upper arms or thighs – in the same procedure or later.
Temporary swelling, bruising, a feeling of tightness and numbness are common. Less often, wound-healing problems, intolerance reactions, thrombosis, unevenness or asymmetries occur, which may require a revision. In addition, there are the general risks of any anaesthesia; you will be given detailed information about these beforehand.
Not for aesthetic reasons. Lipoedema is different: there, liposuction is covered by statutory health insurance under certain conditions – with its own treatment pathway.
