Lipoedema
More than a matter of shape.
Lipoedema is a chronic disorder of the fat tissue that affects almost exclusively women. The legs, and often the arms, increase in circumference symmetrically, while the hands and feet remain slim. Many of those affected describe heaviness, tightness and pain even under light pressure.
Diet and exercise have little effect on the diseased fat tissue. Compression, lymphatic drainage and exercise relieve the symptoms – it can only be removed surgically, by liposuction.
Not every fat deposit on the legs is lipoedema. Distinguishing it from lymphoedema and obesity belongs in the hands of specialists.
The treatment
What the operation can achieve.
Lasting
Fat cells that have been removed do not grow back. With a stable weight, the result is maintained.
Gentle
The tumescent technique with water-jet-assisted or vibration-assisted liposuction (WAL, PAL) spares the surrounding tissue.
Mobile
Less volume and less pain mean more movement in everyday life for many patients.
For treatment covered by statutory health insurance, a fixed process with a four-eyes principle applies: whoever makes the diagnosis does not perform the operation.
First appointment
A specialist in plastic and aesthetic surgery records the findings and makes the provisional diagnosis.
Requirements
The patient management team checks which requirements for the operation are already met and what is still missing.
Preparation
If necessary, conservative therapy begins: compression, lymphatic drainage and exercise, for six months and documented without gaps.
Confirmed diagnosis
It is made by a specialist in their own practice, for example in angiology, dermatology or phlebology – independently of the operation.
Indication
The operating specialist determines the final indication and plans the procedure.
Surgery
Liposuction using the tumescent technique – depending on the extent, as an outpatient or with inpatient monitoring.
Aftercare
Compression, lymphatic drainage and follow-up appointments support healing, with skin tightening later if desired.

Statutory health insurance

Since the decision of the Federal Joint Committee (G-BA) of 17 July 2025, liposuction for lipoedema has been covered by German statutory health insurance regardless of the stage, provided these conditions are met:
a confirmed diagnosis by specialists qualified to make it, separate from the operation
six months of conservative therapy, prescribed by a doctor and documented
a stable weight in the six months before the procedure
a BMI below 35; between 32 and 35, the ratio of waist to height must also be suitable
Procedures on the legs, arms, hips and buttocks are covered; skin tightening is not. The operation takes place at the Bürgerspital Wertheim; preparation and aftercare are also possible close to home at the other locations in Germany.
Self-pay patients
Without six months of prior treatment.
If you pay for treatment yourself, you do not need six months of conservative prior treatment – the path to surgery is shorter. This is possible at all five locations.
Skin tightening after liposuction can be planned at the same time if you wish. The costs depend on the extent and number of procedures; you will receive a quote after the first consultation.

Does statutory health insurance cover liposuction for lipoedema?
Since the decision of the Federal Joint Committee (G-BA) of 17 July 2025, it has been covered by German statutory health insurance regardless of the stage, provided the requirements are met: a confirmed diagnosis under the four-eyes principle, six months of conservative therapy, a stable weight and a BMI below 35. Statutory insurance does not cover skin tightening.
Do I need approval from my health insurer beforehand?
No. If the requirements are met, liposuction is a standard benefit; there is no separate application to the health insurer. The necessary documents are produced along the way to the operation.
Who may make the diagnosis?
For treatment covered by statutory health insurance: specialists in internal medicine and angiology, in physical and rehabilitation medicine, in dermatology, or with the additional qualification in phlebology. Whoever operates does not make the diagnosis themselves – that is the four-eyes principle.
How many procedures are needed?
Usually one to three, a few weeks apart. It depends on how many areas of the body are affected.
Does the fat come back?
Fat cells that have been removed do not grow back. With major hormonal changes or significant weight gain, however, the remaining fat cells can increase in volume – a stable weight secures the result.
How long does recovery take?
Swelling and a feeling of pressure usually subside within one to two weeks. Compression is worn for six to eight weeks; office work is often possible after seven to ten days, sport after medical clearance, usually after four to six weeks.
Is liposuction for lipoedema the same as cosmetic liposuction?
The technique is related, the aim is not: with lipoedema, the goal is to remove pathologically altered fat tissue and relieve symptoms – not to change the shape of the body.
