The eye area
The skin of the eyelids is very thin and is among the first to lose its elasticity over the years. On the upper lid, excess skin folds over the eyelid crease, creating hooded eyelids. On the lower lid, the connective tissue gives way and fatty tissue bulges forwards – the so-called eye bags.
Besides age, genetic predisposition and weak connective tissue play a role. Hooded eyelids can get in the way when applying make-up and, if they droop a long way, restrict the upper field of vision. Some of those affected then report tired eyes and headaches.
Not every dark shadow under the eye is an eye bag. Pigmentation, visible blood vessels or a tear trough look similar but need a different treatment. The examination clarifies what lies behind it.
The treatment
Excess skin and fat are removed through an incision in the eyelid crease, together with a narrow strip of muscle if needed. The scar lies in the natural crease and is usually barely visible once healed.
The incision lies just below the lash line. Fat can be removed or moved into the groove between lid and cheek; if the skin is still firm, access from the inside of the lid is often enough.
Afterwards, the eye area can look more alert and rested. If the upper lids droop heavily, the field of vision may also widen.

Eyelid surgery is a short procedure that is planned precisely: the excess skin is measured and marked beforehand.
A specialist in plastic and aesthetic surgery examines the lids, brows and skin and clarifies whether the upper lids, the lower lids or both will be treated. Dry eyes or eye conditions call for a particularly careful assessment.
Blood thinners, alcohol and smoking need attention a few days beforehand; sunbeds and long sunbathing sessions are stopped about a week before.
Usually under local anaesthetic, with twilight sedation if desired. Upper eyelid surgery takes about 45 to 60 minutes, lower eyelid surgery often longer.
Immediately afterwards and during the first few days, the lids are cooled regularly; this helps against swelling and bruising. After upper eyelid surgery, patients usually go home the same day.
The stitches are removed after about a week. No driving in the first week, and no contact lenses or eye cream for two weeks.
After one to two weeks, swelling and discolouration have usually subsided enough for any remaining traces to be covered with make-up. Avoid sun and sunbeds for about four weeks.

If the aim is cosmetic, you pay the costs yourself. The price depends on whether the upper lids, the lower lids or both are treated and whether the procedure is combined with another, such as a facelift.
You receive a personal quote after the preliminary consultation; several clinics offer financing in instalments.
Medical indication
If the upper lids droop so far that they restrict vision, there may be a medical indication. Whether German statutory health insurance then covers the costs is assessed case by case; the basis is an ophthalmologist’s report on the field of vision.

The local anaesthetic is given with a fine needle; after that, most people only feel touch and pulling. In the days that follow, the lids often feel tight. Discomfort can be treated well with cooling and mild painkillers.
On the upper lid, the scar lies in the eyelid crease; on the lower lid, just below the lashes. Once healed, both are usually barely visible. With access through the inside of the lower lid, there is no external scar.
After about one to two weeks. The stitches are removed after around a week; any remaining discolouration can then usually be covered with make-up.
