The cleft
In a cleft lip and palate, the upper lip, jaw or palate do not fully fuse during development before birth. The cleft can be on one side or both sides and can affect individual areas or all of them. It is one of the more common congenital differences of the face.
Treatment usually begins in the first months of life and accompanies children for years – with operations, orthodontics and speech therapy. The nose is often affected too: the columella (the strip of skin between the nostrils), the nostrils and the septum are unevenly formed, one side of the nose is narrower and breathing is more difficult.
Once growth is complete, some people are left with unfulfilled wishes: to breathe more freely, a more balanced nose, a less conspicuous scar on the upper lip, a palate that interferes less with speaking and swallowing. Corrections in adolescence and adulthood exist for this – even after many previous operations.
The treatment
A straightened septum and supported nostril walls can make nasal breathing easier.
The tip of the nose, the columella and the nostrils can be brought into balance with one another – as far as the tissue and scars allow.
Procedures on the palate can help if speech or swallowing is still impaired.

Nasal breathing, the nasal framework, scars and palate are examined closely – together with other specialties such as ENT, orthodontics or speech therapy if needed.
There are often several goals: free breathing, a balanced shape, softer scars. Together, you decide what makes sense and in what order.
The correction is performed under general anaesthesia and takes four to six hours, depending on the findings. The patient’s own cartilage often supports the nasal framework.
A splint protects the nose for one to two weeks, and most patients go home after two to three nights. Swelling subsides over several weeks; the final result becomes apparent after several months.
Follow-up appointments check healing and breathing. Sometimes a further, smaller procedure makes sense later on.

A cleft lip and palate is congenital, and its effects often extend into adulthood. If a correction is medically necessary, for example because of obstructed nasal breathing or speech difficulties, German statutory health insurance may cover the costs. This is assessed case by case.
