Breast augmentation
There are many reasons for wanting fuller breasts: breasts that never developed as hoped, two breasts of different sizes, or volume that was lost after pregnancy, breastfeeding or major weight loss.
The benchmark is not a particular cup size but the proportion between breasts, chest and shoulders. The width of the breast base, the elasticity of the skin and the thickness of the tissue determine which implant makes sense and where its limits lie. If the breasts sag noticeably, volume alone is often not enough – a lift then becomes part of the plan.
Alongside conventional breast augmentation with implants, there are two other options: the tissue-sparing Preservé™ breast augmentation by Motiva®, in which the implant pocket is stretched rather than surgically dissected, and breast augmentation with fat transfer, which requires no implant and allows a moderate enlargement.
The treatment
The width, projection and volume of the implant are determined in advance. This means the change can be set anywhere from subtle to pronounced.
Round or anatomical shape, placement above or partly below the chest muscle: the combination depends on your tissue and the look you want.
With a lift if the breasts sag, or with your own fat, which can soften the transitions where the tissue covering is thin.

Between the preliminary consultation and the operation, there is time to reflect. It is part of the process – breast augmentation is a decision that deserves calm consideration.
A specialist in plastic and aesthetic surgery examines the breasts, measures the breast base and skin elasticity and discusses your ideas – including what is not possible.
The implant shape, size, position and access route are decided. The incision is most often made in the fold beneath the breast; the edge of the areola and the armpit are also possible.
After you have been informed about the risks and anaesthesia, there is time to think everything over calmly. The written quote follows the preliminary consultation.
Under general anaesthesia, the pocket for the implant is created, the implant is inserted and the wound is closed in layers. The procedure usually takes one to two hours.
Usually one night in the clinic. A feeling of tightness, swelling and restricted arm movement are normal at first and can be relieved with medication.
The support bra is worn for four to six weeks, and sport and heavy lifting are paused for the same period. The stitches come out after about two weeks; the scars need protection from sun and sunbeds.
The shape settles over weeks to months. Regular check-ups, with ultrasound or MRI if needed, accompany the implant in the longer term too.

Self-pay treatment
Patients pay for breast augmentation for aesthetic reasons themselves. The price depends on the implant, the length of the operation and whether a lift or the correction of an asymmetry is also needed.
You will receive a personal quote after the preliminary consultation, once the implant, scope and anaesthesia have been decided. Several clinics offer financing in instalments.
Health insurance
German statutory health insurance does not cover breast augmentation for aesthetic reasons. In the case of pronounced congenital malformations of the breast, it is assessed case by case whether cost coverage may be possible.
For the treatment of consequences of an aesthetic operation, such as capsular contracture, statutory insurers require insured persons to contribute to the costs under § 52 SGB V (German Social Code, Book V). Insurance for follow-up costs can cover such procedures, depending on the policy.

Silicone gel implants, round or anatomically shaped, in different widths, projections and surfaces. Each clinic shows on its own website which manufacturer it uses. More important than the volume in millilitres is that the width and projection suit the breast.
Usually in the fold beneath the breast, where it is barely noticeable once healed. Alternatives are the edge of the areola and the armpit. Every scar matures over months and becomes paler over time; sun protection and scar care help with this.
Above the muscle, the décolletage often looks more defined and recovery is shorter. Partly below the muscle (dual plane), more tissue covers the upper edge of the implant, which can soften the transitions in slim women. The decision follows the tissue.
As with any operation: post-operative bleeding, infection and wound-healing problems. Specific to implants are, above all, capsular contracture – a hardening of the tissue around the implant – as well as changes in position and rippling. Very rare is a form of lymphoma (BIA-ALCL) that has been linked to certain textured surfaces. All risks are covered in the informed consent discussion.
In many cases, yes. The access route, implant position and your own anatomy are decisive; it cannot be predicted with certainty. If you are planning to have children, raise this openly at the preliminary consultation.
There is no fixed date for replacement. Modern implants are durable; they are replaced if symptoms, a defect or the wish for a different size arise.
Only once the breasts are fully developed and the patient is of legal age. A purely aesthetic operation requires a mature decision of one’s own – and time to think it over.
Implants are suitable if more volume is desired or there is little fat of your own. Preservé™ is a tissue-sparing technique, mainly for first-time procedures where no lift is needed. Fat transfer enlarges the breasts by about half a cup size to one cup size per session and requires no implant. The examination shows which method is right.
